Mentorship Program Application
A completed registration application is required from each program participant. It is recommended that you submit your application as soon as possible. We expect the programs will sell out quickly and registrations are processed on a first-come, first-served basis.
You may complete the registration form and pay online via our secure PayPal account or you may print a registration form and submit payment via fax or mail. When completing the registration form, please indicate your choice for program dates. Incomplete registration forms will not be processed and may delay program reservations.
Registration Acknowledgement
You will receive an acknowledgment email within 5 business days notifying you of payment receipt and seating status. Programs are normally confirmed or cancelled based on enrollment approximately 30 business days before the scheduled start date. Please consider this when purchasing nonrefundable airline tickets.
Program Changes/Cancellations
It is sometimes necessary to change the dates a program is offered or to cancel a program. Participants are contacted at the earliest opportunity in the event of a scheduling change or cancellation. However, changes or cancellations may be made up to 30 business days prior to the program start. Please consider this when purchasing nonrefundable airline tickets. If the program is cancelled, all payments will be refunded including the $250 program deposit.
Participant Transfers/Cancellations
The tuition amount includes a $250 nonrefundable administrative fee per person per program. SPD Foundation reserves the right to charge this fee for multiple participant transfers. Participant substitutions (a participant's place in program is filled by another person from the same organization) are not allowed without prior approval by the SPD Foundation staff. The tuition less the administrative fee will be refunded if SPD Foundation receives notice of cancellation at least 30 days prior to the program start date. The full tuition will be charged to participants who cancel less than 30 days before the program start or who fail to appear for the program.
Transportation
Transportation to the training site is the participant's responsibility. SPD Foundation assumes no responsibility for nonrefundable travel arrangement losses resulting from program scheduling changes or cancellations.
Food and Lodging
Participants are provided with a list of area hotels and area restaurants. Participants are responsible for making their own lodging arrangements. Meals are not provided.
Course Materials
All course materials are provided at the training site.
Recommended Attire
Professional but comfortable/casual dress is suggested since time will be spent in the therapy room with children.
